Tucker Wellness & Rehab is seeking a dedicated Physical Therapist (PT) or Physical Therapist Assistant (PTA) to join our exceptional in-house rehabilitation team.
Under new ownership and management, Tucker Wellness & Rehab is a 136-bed skilled nursing and rehabilitation center providing both short-term rehabilitation and long-term care services. Our newly renovated therapy gym and facility create an outstanding environment for delivering high-quality patient care.
Physical Therapist (PT) – Full-Time
Pay: $50.00 - $65.00 per hour (up to $120,000 annually)
Physical Therapist Assistant (PTA)
Pay: $35.00 - $40.00 per hour
Physical Therapist (PT) Responsibilities
- Screen, evaluate, and develop individualized treatment plans for residents.
- Assess mobility, strength, balance, endurance, pain, and functional limitations.
- Provide evidence-based treatment interventions to restore function and improve quality of life.
- Supervise Physical Therapist Assistants (PTAs) in accordance with Georgia practice standards.
- Record daily treatment charges following approved billing guidelines.
- Complete evaluations, daily documentation, progress notes, and discharge summaries accurately and timely.
- Participate in resident care conferences, interdisciplinary team meetings, and discharge planning.
- Communicate resident progress and treatment recommendations with physicians, nursing staff, families, and the rehabilitation team.
- Provide caregiver and family education to support safe discharge and continued progress.
- Participate in facility in-service education and quality improvement initiatives.
- Coordinate with members of other departments to ensure comprehensive patient care.
- Perform other duties as assigned.
Physical Therapist Assistant (PTA) Responsibilities
- Assist the supervising Physical Therapist in implementing individualized treatment plans.
- Provide therapeutic exercises, gait training, balance training, transfer training, and other prescribed interventions.
- Monitor resident progress and communicate changes to the supervising therapist.
- Accurately document treatments and resident responses.
- Educate residents on proper exercise techniques and safety.
- Collaborate with therapists, nursing staff, physicians, and families to support resident goals.
- Maintain a safe, clean, and organized therapy environment.
- Perform other duties as assigned.
QualificationsPhysical Therapist (PT)
- Current Physical Therapist license in the State of Georgia.
- CPR certification preferred.
- Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program.
- Skilled nursing or rehabilitation experience preferred.
- Strong knowledge of anatomy, physiology, and medical terminology.
- Excellent communication, documentation, and organizational skills.
- Ability to supervise support staff and work effectively within an interdisciplinary team.
Physical Therapist Assistant (PTA)
- Current Georgia Physical Therapist Assistant license in good standing.
- Skilled nursing or rehabilitation experience preferred.
- Knowledge of anatomy, physiology, and rehabilitation principles.
- Strong communication and teamwork skills.
- Compassionate, dependable, and resident-focused.
- Excellent documentation and organizational abilities.
