Schedule: PRN/Per-Diem (Coverage needs typically Monday–Friday, 8:00 AM–5:00 PM)

Bristol Hospice is seeking a compassionate and experienced Hospice Social Worker to support patients and families throughout Lake, Geauga, Medina, and Cuyahoga Counties. This is an ideal opportunity for a social worker who values autonomy, enjoys building relationships in the community, and is comfortable traveling throughout the service area to make an impact where it's needed most.
Hospice experience is required. We're looking for someone who understands the unique emotional, psychosocial, and practical challenges that patients and families face at end of life—and who can step in with empathy, expertise, and support.

You'll help families navigate difficult conversations, connect them with valuable resources, provide crisis intervention when needed, and work alongside an interdisciplinary team dedicated to delivering exceptional end-of-life care.

What You'll Do
- Conduct psychosocial assessments of patients and families
- Provide emotional support, counseling, and crisis intervention
- Help families navigate stress, grief, loss, and end-of-life decision-making
- Educate patients and caregivers regarding advance directives and care planning
- Connect families with community resources and support services
- Act as a liaison between patients, families, caregivers, and community agencies
- Collaborate closely with nurses, chaplains, physicians, and the hospice care team
- Participate in interdisciplinary team meetings and contribute to individualized care plans
- Assist families with planning related to financial, legal, and funeral arrangements
- Maintain accurate and timely clinical documentation
- Support quality improvement initiatives and patient-centered care

Requirements:
Required:
- Bachelor's or Master's degree in Social Work from an accredited program, or a bachelor's degree in a related field that meets Medicare hospice qualification requirements. Please know the hirable rate of pay will be based on degree type.
- Must have minimum of two (2) years of documented supervised experience in health care, hospice experience preferred
- Current Ohio LSW or LISW license required.
Preferred: Master of Social Work (MSW) degree.
- Must understand hospice philosophy, and issues of death/dying.
- Certified Hospice and Palliative Social Worker is desirable
- Must be flexible in work hours and have the ability to travel throughout the assigned Bristol Hospice service area
- Must demonstrate a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs
