Registered Nurse (RN) – Home Care Supervisor (Part-Time / As Needed)

Job Summary
We are seeking a compassionate, organized, and experienced Registered Nurse (RN) to join our growing home care agency. This is a part-time, as-needed position. As our agency expands and our client base grows, additional hours may become available.
The RN will provide clinical oversight for non-medical home care services, conduct client assessments, develop individualized care plans, supervise caregivers, and ensure compliance with New Jersey regulations and agency policies.
Responsibilities
- Conduct initial in-home client assessments.
- Develop, implement, and update individualized care plans.
- Complete 30-day supervisory phone calls with clients.
- Conduct required 60-day in-home supervisory visits.
- Perform six-month client reassessments.
- Evaluate clients for changes in condition and recommend updates to the care plan.
- Supervise Certified Home Health Aides (CHHAs) and ensure quality of care.
- Maintain accurate and timely clinical documentation.
- Collaborate with clients, families, caregivers, and agency staff.
- Ensure compliance with New Jersey home care regulations.
- Participate in quality assurance and clinical oversight activities.
- Be available to provide clinical guidance and support when needed.
QualificationsRequired
- Active, unrestricted Registered Nurse (RN) license in the State of New Jersey.
- Minimum 2 years of nursing experience.
- Current CPR/BLS certification.
- Valid driver's license and reliable transportation.
- Ability to travel to client homes within New Jersey.
- Strong assessment, communication, and documentation skills.
- Ability to work independently and manage time effectively.
Preferred
- Home care, home health, hospice, case management, or community nursing experience.
- Experience supervising caregivers or home health aides.
- Familiarity with electronic medical records (EMR).
