Duties
- Personal care and assistance with activities of daily living. Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support.
- Monitor patient health by checking vital signs, observing symptoms, and reporting changes to healthcare professionals.
- Mobility assistance and fall prevention
- Medication reminders following prescribed care plans while adhering to HIPAA regulations for patient privacy.
- Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary.
- Companionship and meaningful conversation
- Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning patient areas and ensuring infection control protocols are followed.
- Transportation to appointments and errands (when needed)
- Document care activities accurately in compliance with healthcare standards and facility policies. Accurate documentation using our mobile care app
Skills
- Proven experience working with individuals with developmental disabilities, autism, dementia, or neurodevelopmental disorders.
- Strong patient observation skills to detect changes in condition or behavior promptly.
- Knowledge of vital signs measurement, and basic first aid including CPR certification.
- Familiarity with care plans and documentation procedures while maintaining HIPAA compliance.
- Experience in group homes, assisted living facilities, nursing homes, or home health environments is highly desirable.
- Excellent communication skills for effective interaction with residents, families, and healthcare teams.
