Position Summary
The Registered Nurse (RN) provides coordinated physical and behavioral health nursing services within an integrated outpatient clinic model. The RN supports patient assessment, care coordination, medication management, crisis response, clinical monitoring, diagnostic testing, and interdisciplinary collaboration to ensure high-quality, patient-centered care across mental health, substance use, rehabilitation, and medical service lines.
The RN plays a critical role in supporting crisis services, adult stabilization and observation, chronic condition monitoring, opioid treatment coordination, pain management support, and CLIA-waived laboratory services.
Essential Duties & Responsibilities
Clinical Nursing Care

Conduct nursing assessments including vital signs, health histories, and symptom monitoring

Monitor patients during crisis interventions and adult stabilization/observation services

Provide clinical support during psychiatric evaluations and medical visits

Administer medications as ordered and monitor for adverse effects

Perform wound care and basic medical procedures within scope of practice
Integrated Care Coordination

Collaborate with behavioral health providers, psychiatric prescribers, rehabilitation therapists, and case managers

Support treatment planning and interdisciplinary case conferences
-
Coordinate care transitions following crisis or hospitalization
-
Provide patient education related to medications, health conditions, and wellness
CLIA-Waived & Clinical Testing Support

Perform CLIA-waived point-of-care testing (urinalysis, glucose, pregnancy, rapid tests, drug screening)

Ensure proper specimen handling and documentation

Follow infection control and laboratory compliance procedures
Crisis & Stabilization Support

Assist with suicide risk assessments and safety planning
-
Provide de-escalation support during behavioral health crises

Monitor patients for clinical stability and safety

Document crisis interventions and outcomes
Physical Health & Chronic Condition Support

Monitor chronic medical conditions (e.g., diabetes, hypertension)

Support pain management care plans

Conduct preventive health screenings

Perform medication reconciliation
Opioid & SUD Care Coordination

Support MAT coordination and monitoring

Conduct nursing follow-ups for substance use treatment

Monitor withdrawal symptoms when applicable

Provide recovery education and medication adherence support
Compliance & Documentation

Maintain accurate clinical documentation in the EHR

Follow HIPAA, 42 CFR Part 2, and clinic policies

Participate in quality assurance and audits

Ensure scope-of-practice compliance
Qualifications
Required
-
Active Arizona Registered Nurse (RN) license
-
Graduate of an accredited nursing program
-
CPR/BLS certification
-
Experience in outpatient, behavioral health, primary care, or integrated care settings

Strong clinical assessment and documentation skills
Preferred

Behavioral health or crisis care experience

Substance use disorder treatment experience

Care coordination or case management exposure

Familiarity with AHCCCS or managed care environments

Experience with CLIA-waived testing

The Registered Nurse (RN) provides comprehensive nursing care within an integrated outpatient clinic, supporting both physical and behavioral health services. The RN plays a key role in patient assessment, care coordination, medication management, and clinical monitoring across mental health, substance use, rehabilitation, and medical services.
The RN is also instrumental in crisis stabilization and intervention, including monitoring patients during behavioral health crises, supporting suicide risk assessments and safety planning, assisting with de-escalation, and ensuring patient safety during adult stabilization and observation services. The RN collaborates closely with the interdisciplinary team to support continuity of care, particularly during transitions following crisis episodes or hospitalization.
In addition, the RN supports chronic disease management, opioid and substance use treatment coordination (including MAT), pain management, and CLIA-waived diagnostic testing, while maintaining compliance with all regulatory and documentation requirements.
Overall, the RN ensures delivery of safe, coordinated, and patient-centered care, with a strong focus on clinical stability, crisis response, and integrated health outcomes.
