Heyl Family Practice is currently seeking a Full-Time CRNP or PA to join our team of providers. This medical professional will provide short-term and long-term healthcare services for nursing home patients and personal care patients with certain acute or chronic health conditions in accordance with applicable scope and standards of practice and with the policies, values, and mission of our organization. This professional will also rotate weekend and holiday hospital coverage.
This position would require the desired candidate to be responsible for servicing nursing home patients at a single nursing home we support. The hours for this position would be daylight and provide some slight flexibility, but would require specific patients to receive support each week. This would be a full-time position with direct, in-person contact with patients.

Led by Dr. Donald Shoenthal, Dr. Scott Heyl, Dr. Matthew Macken, Dr. Brian Nolen, and Dr Joseph Martinez, Heyl Family Practice is a local team of primary care providers located north of Pittsburgh, providing a variety of immediate and long-term medical services for patients at every point of their healthcare journey. The goal of the practice is to ensure continuity of care and to provide a supportive relationship between the patient and their provider.
Currently, our practice includes a large team of the five leading physicians and small teams of Medical Assistants (MAs), Certified Registered Nurse Practitioners (CRNPs) and Physician Assistants (PA-C) to support our robust patient community. Of our team of providers, our facility providers (CRNPs and PA-Cs) share responsibility supporting various nursing homes and rounding at Passavant Hospital to support patients.
Essential Responsibilities:
- Provides appropriate patient care to include recording patient medical history and physical assessment, developing therapeutic care plan, ordering diagnostic tests, interpreting test results, discharge planning and providing accurate medical record documentation.
- Communicates and coordinates patient care with patient, family, community health services and interdisciplinary team. Involves attending physician regarding plan of care and involves physician in clinical decision making of any issues that are outside the professional's knowledge base or scope of practice.
- Prepares written prescription orders for drugs and controlled substances, administers medication and injections.
- Educates patients and family members regarding health and preventive medicine, medications prescribed, treatment instructions and recommends community resources as appropriate.
- Triages patient telephone calls, provides monitoring and continuity of care.
- Participates, develops, and implements performance and/or quality improvement initiatives. Ensures compliance with all regulatory agencies.
- Serves as the clinical expert and acts as a clinical resource.
- May perform other duties such as daily rounding of patients, manage outpatient concerns and any other duty specific to specialty.
Location: You would be assigned to a specific set of nursing home and personal care homes and, as necessary, utilized to provide coverage at other facilities

Pay: $100,000.00 - $120,000.00 per year
