Job Title: Case Manager & Volunteer Coordinator

Job Summary:
The Case Manager & Volunteer Coordinator provides individualized patient care planning and community resource coordination while overseeing the recruitment, training, and stewardship of the clinic's volunteer program. Working closely with the Director of Clinical Services, this position ensures that every patient receives compassionate, connected support — and that the dedicated volunteers who make that care possible are well-prepared and valued.
Duties/Responsibilities:
Case Management
· Identify patient needs through self-assessment and/or referral; establish a care plan within a variety of social contexts, matching community resources with patient needs
· Coordinate client care with team members to ensure individualized service plans are implemented and patient needs are met through service delivery
· Have knowledge of referral sources network and relevant information to effectively link patients with health systems and community-based service organizations
· Work in collaboration with health professionals to develop an overall health plan specific to the patient’s needs
· Assist patients in addressing emotional and behavioral health needs
· Help individuals with enrollment of appropriate insurance programs best suited to individual patients and resources for their enrollment of appropriate insurance programs
· Coordinate care with other service organizations
· Oversee responsibilities and work closely with Intake Coordinators, ensuring accuracy in patient information and record-keeping
· Coordinate Sharing Dignity (feminine hygiene program)
· Oversee patients’ transition out of the clinic to insurance, when needed
· Keep current regarding changes impacting the ACA, BadgerCare, and other public assistance programs
· Record and report appropriate and accurate data to demonstrate program effectiveness
· Maintain complete and accurate case files for all individuals served
· Provide supervision, training, and guidance to the Hispanic Case Manager
Volunteer Coordination
· Recruit, orient, and schedule volunteers, ensuring that all positions are filled at recommended capacity
· Implement a volunteer stewardship plan to include surveys and formal volunteer recognition
· Accept and respond to all volunteer inquiries in a timely manner, including website, email, and phone inquiries
· Assure proper orientation and training of volunteers to include specifics of jobs and safety concerns
· Maintain volunteer records including contact information and donated hours
· Recruit volunteers as needed for special projects and events
· Coordinate with staff members to fill ancillary clinic and administrative needs
· Promote team culture in all working relationships with staff and volunteers; provide cross-staff support as needed
· Serve as CVFC representative for community events and volunteer fairs
· Other duties as requested
Clinic Support:
Maintain a consistent and reliable presence during clinic operations in accordance with the established schedule, providing direct support to ensure efficient and compassionate patient care.
Supervisory Responsibilities:
This position supervises the Hispanic Case Manager and Intake Volunteers, with responsibility for training, scheduling, and ongoing guidance to ensure consistent, high-quality service delivery.
Required Skills/Abilities:
· Strong verbal and written communication skills
· Fluent in Spanish – written and verbal
· Ability to work independently and within a team structure
· Strong organizational skills with the ability to prioritize
· Working knowledge of various software and database technology, including Microsoft Office and EMR systems
· Working knowledge of Wisconsin and county-based insurance programs, health care, and social services systems
· Knowledge of and experience working with a wide spectrum of individual issues, including alcoholism, drug abuse, and mental health
· Demonstrates respect for diversity
· Awareness of psychosocial barriers that impact the lives of the uninsured
· Adhere to confidentiality policies with patient’s financial and personal records

Education and Experience:
· Bachelor’s degree in social work, case management, or a related field required
· Experience as CHW, Case Management or a related field
· Experience in volunteer coordination, community health, or a clinical setting strongly preferred
Work Environment & Physical Demands
· Clinical/medical office setting with moderate noise levels
· Potential exposure to infectious diseases and biohazard materials
· Frequent bending, standing, walking, and active listening throughout the clinic
· Ability to lift to 15 lbs.
· Regular interaction with patients experiencing illness, emotional distress, or language barriers
Pay: From $25.00 per hour
