Responsibilities:
As a Home Health Nurse, you will be responsible for:
- Delivering high-quality nursing care in accordance with physician orders and established procedures.
- Conducting thorough physical and psychological assessments of patients and developing appropriate nursing interventions.
- Utilizing a problem-solving approach to patient care, considering each patient's unique background, financial situation, and available resources.
- Effectively communicating with patients, their families, physicians, and other healthcare professionals.
- Managing medical and situational emergencies with ingenuity and sound judgment.
- Ensuring the safety of yourself, patients, and other staff members by assessing potential physical and psychological hazards in home environments.
- Coping effectively with individuals from diverse backgrounds, socioeconomic conditions, and value judgments.
Qualifications:
- Current and valid LVN or RN license in Texas.
- Current BLS/CPR certification
- Applicants should have at least one year of home health experience with proven success and knowledge or admissions and recertifications.
- Knowledge of Axxess Technology Solution is required.
- Ability to follow instructions meticulously from physicians, nursing supervisors, and other professional staff.
- Reliable transportation to make patient home visits as needed.
- Strong assessment skills, including the ability to identify both physical and psychological patient needs.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Must be able to assess safety of home situations for self, patients, and other staff members.
- Ability to effectively cope with patients, families, and others with varying backgrounds and situations.
