Hayat Care Center, Inc. is a community-focused Adult Day Care and Home Care organization dedicated to providing compassionate, high-quality care to adults and seniors in Colorado Springs. We're growing our team and looking for a dedicated Case Manager to help coordinate care and support our clients and their families.
Position Summary
We are seeking an experienced and compassionate Case Manager to oversee client care coordination across our Home Care and Adult Day Care programs. The ideal candidate will manage client intake, develop and monitor individualized care plans, coordinate with families and healthcare providers, and ensure compliance with state and program regulations (including HCBS/IHSS standards).
Key Responsibilities
- Conduct client intake assessments and develop individualized care plans
- Coordinate services between clients, families, caregivers, and healthcare providers
- Monitor and update care plans based on client needs and progress
- Ensure documentation is accurate, complete, and compliant with state/CDPHE regulations
- Serve as a liaison between clients/families and internal care staff
- Conduct regular client check-ins and home/facility visits as needed
- Maintain accurate records in the agency's electronic records platform
- Support quality assurance and compliance efforts related to licensing and certification
- Respond to client and family concerns in a timely, professional manner
Qualifications
- Bachelor's degree in Social Work, Human Services, Nursing, or related field (or equivalent case management experience)
- Prior experience in case management, home care, or adult day care settings preferred
- Familiarity with HCBS/IHSS programs and Colorado care regulations a plus
- Strong organizational, communication, and interpersonal skills
- Ability to manage multiple client cases and prioritize effectively
- Proficiency with electronic health/records systems
- Reliable transportation for home visits, if applicable
- Must pass background check and meet state licensing requirements for care staff
