Job description:
We are seeking compassionate and dependable Registered Nurses (RN) with at least one year of home health experience to provide skilled nursing visits to patients in the Grayslake and Evanston service areas. The ideal candidate is organized, clinically competent, and committed to delivering high-quality, patient-centered care in the home setting.
Responsibilities
- Perform skilled nursing visits in patients' homes according to physician orders and agency policies.
- Complete comprehensive patient assessments and ongoing evaluations.
- Provide wound care, catheter care, medication management, injections, and other skilled nursing procedures.
- Monitor patient condition and promptly report changes to the physician and clinical supervisor.
- Educate patients and caregivers on disease management, medications, safety, and infection prevention.
- Accurately complete OASIS assessments and electronic documentation within required timeframes.
- Coordinate care with physicians, therapists, and interdisciplinary team members.
- Maintain compliance with Medicare Conditions of Participation and agency standards.
Qualifications
- Current Illinois RN or LPN license in good standing.
- Minimum of one (1) year of recent home health experience required.
- Current BLS/CPR certification.
- Valid driver's license, reliable transportation, and current auto insurance.
- Strong assessment, communication, and time management skills.
- Experience with electronic medical records (EMR) and OASIS documentation preferred.
- Ability to work independently and manage a flexible schedule.
