We are seeking a compassionate and dedicated Medical Social Worker specializing in hospice care to join our multidisciplinary team. Responsibilities
- Conduct comprehensive patient assessments to evaluate psychosocial needs, including mental health status, family dynamics, and community resources.
- Develop individualized care plans aligned with clinical ethical guidelines and social work practice standards, focusing on end-of-life care and patient-centered goals.
- Provide direct clinical counseling, including individual, family, and group therapy sessions utilizing techniques such as motivational interviewing, cognitive behavioral therapy, and behavioral health interventions.
- Coordinate with healthcare teams to facilitate discharge planning, home care arrangements, and community resource linkage for seamless patient transitions.
- Document all medical records accurately within electronic medical record (EMR) systems while adhering to HIPAA regulations to ensure confidentiality and compliance.
- Engage in crisis intervention when necessary, managing urgent situations with sensitivity and professionalism.
- Support patients with diverse needs—including those with dementia, Alzheimer’s disease, autism spectrum disorder, or developmental disabilities—delivering specialized hospice and palliative care services.
Experience
- Proven experience in medical social work within hospice or palliative medicine settings is an asset but not a requirement .
- Demonstrated knowledge of hospice & palliative medicine practices, including end-of-life care protocols and Medicare regulations, also not a requirement but beneficial.
- Prior inpatient or home & community care experience is highly desirable; familiarity with assisted living or nursing home environments is a plus.
- Strong background in working with individuals and diverse conditions such as dementia, or disabilities.
- Competency in clinical research methods, data collection techniques is advantageous.
- Familiarity with EMR systems and medical terminology ensures accurate documentation and efficient workflow management.

Pay: $55,000.00 - $75,000.00 per year
