The Lakes Home Care is currently seeking a dedicated and passionate Speech Therapist (Speech-Language Pathologist) to join our growing team. The ideal candidate will assess, diagnose, and treat speech, language, voice, cognitive-communication, and swallowing disorders across various age groups, including neonatal, pediatric, adolescent, adult, and geriatric populations.
Conduct comprehensive evaluations and assessments of patients with speech, language, cognitive, and swallowing disorders
Develop individualized treatment plans based on patient evaluations and diagnosis
Provide direct therapeutic interventions using evidence-based practices
Monitor and document patient progress, adjusting treatment plans as needed
Educate patients and families on communication strategies and home programs
Collaborate with physicians, occupational therapists, physical therapists, and other healthcare professionals to ensure holistic care
Supervise and support Speech Therapist Assistants as required by state guidelines
Implement and train on augmentative and alternative communication (AAC) systems
Maintain accurate, timely, and compliant documentation in accordance with professional and regulatory standards
Participate in team meetings and contribute to interdisciplinary care planning
Experience working with pediatric, adult, and geriatric populations in various settings (home health, schools, hospitals, etc.)
Strong understanding of speech-language development, disorders, and intervention techniques
Knowledge of IEPs, IFSPs, and documentation procedures in educational settings (if applicable)
Familiarity with AAC devices and strategies
Ability to adapt treatment to critical care and medically complex environments
Excellent communication, collaboration, and case management skills
Compassionate and patient-centered approach to care
Experience with standardized assessments and outcome measurement tools
Master’s Degree in Speech-Language Pathology from an accredited program
Current and valid State Licensure or Certification in Speech-Language Pathology
Certificate of Clinical Competence (CCC-SLP) from ASHA (or CFY candidates will be considered)
CPR certification (if required by state regulations)
Ability to pass background check and meet health screening requirements
Commitment to continuing education and maintaining licensure
