Registered Nurse (RN) – Home Health – Per Diem / PRN
Our Medicare-certified home health agency is hiring Home Health RN clinicians to provide skilled nursing visits in patients’ homes. We offer flexible per diem/PRN scheduling and are recruiting separately for Fountain Valley/Orange County and Los Angeles coverage. Apply for the region that fits your availability.
Job Description
Provide one-on-one nursing care, assess patient needs, educate patients and caregivers, and coordinate care according to each patient’s Plan of Care. This is a field-based position involving local travel within your agreed-upon service area.
Responsibilities
- Perform skilled nursing visits, patient assessments, and follow-up care.
- Complete Start of Care (SOC) visits and applicable OASIS assessments when qualified and assigned.
- Review medications and educate patients and caregivers on proper use, precautions, and adherence.
- Provide wound care and nursing treatments as ordered and within your competency.
- Monitor patient progress and promptly communicate changes in condition.
- Communicate with physicians regarding patient status, care needs, and orders.
- Coordinate with the interdisciplinary team and follow the patient’s Plan of Care.
- Complete accurate, timely clinical documentation.
- Follow infection-prevention practices, patient-privacy requirements, and agency procedures.
Qualifications
- Active California RN license.
- Current CPR/BLS certification.
- Valid driver’s license and reliable transportation.
- Willingness to travel locally within your selected coverage area.
- Strong assessment, communication, patient education, and documentation skills.
- Home health experience preferred.
- Familiarity with Medicare home health documentation and OASIS preferred, as applicable to assigned visits.
Schedule and Employment Type
- Per Diem / PRN.
- Flexible visit scheduling coordinated around clinician availability, patient needs, and ordered visit frequency.

Fountain Valley / Orange County
We are hiring RNs to cover Fountain Valley, CA and surrounding areas, as well as other Orange County communities.
Los Angeles
We are separately hiring RNs to cover Los Angeles County, including ZIP code 90007 and surrounding Los Angeles areas.
You may apply for either region or both. Fountain Valley/Orange County clinicians are not required to cover Los Angeles, and Los Angeles clinicians are not required to cover Orange County. Home visits are assigned within your agreed-upon service area.
