Havenwood Heritage Heights is seeking a compassionate, motivated Social Worker to join our Care Management Department. This is an exciting opportunity to make a meaningful difference in the lives of older adults while working alongside a dedicated interdisciplinary team.
Responsibilities
- Provide social work services to assisted living, short-term rehabilitation, and long-term care residents.
- Coordinate discharge planning for short-term rehabilitation residents.
- Collaborate with an interdisciplinary care team to support resident well-being.
- Work closely with outside agencies and community resources.
- Assist residents and families with Medicare and Medicaid-related needs.
- Prioritize and manage multiple responsibilities in a fast-paced environment.
- Demonstrate strong organizational skills, independence, and self-motivation.
Preferred Qualifications
- 1–3 years of experience in a hospital or long-term care setting.
- Experience working with individuals living with dementia.
- MSW preferred; BSW candidates will also be considered.
