Case Management Specialist (MHW)
Position Summary: Routinely provides front-line consumer-focused work within the community-based mental health system. Delivers
clinical and supportive services under the supervision of a Team Leader ("MHP") or Qualified Mental Health Professional ("QMHP").
POSITION QUALIFICATIONS:
Minimum Requirements (Meet one):
-Bachelor's (BA): Degree in Social Work, Nursing, Counseling, Psychology, Rehab, Sociology, Human Development (or related field with ≥12 credits) AND 1 year of direct mental
health experience.
- Certifications: Certified Psychiatric Rehabilitation Practitioner (CPRP).
- Equivalencies: Certified Substance Abuse Counselor (CSAC), Certified Peer Specialist (HCPS), RN, or LPN with 12 post-high school human development credits AND 1 year of mental health experience.
- Required Certifications: BLS, CPR, First Aid, and CPI (must maintain active status).
- Competencies: Clinical problem-solving, critical thinking, electronic documentation proficiency, and collaboration
CASELOAD & DOCUMENTATION
- Performance Target: Full-Time produces 450 units; Part-Time produces 200 units of direct consumer contact per month.
- Face-to-Face: ≥75% of all consumer contacts must be face to-face.
- Strict Charting Window: All face-to-face, collateral, and telephone contacts must be documented in progress notes within 24 hours. Inter-team/pharmacy notes logged in general notes.
SUPERVISION & TRAINING
- High School/GED Staff: Weekly QMHP supervision; all Recovery Plans and Progress notes must be co-signed by QMHP.
- BA Level Staff: Clinical supervision with Team Leader ≥3 times/month (individual, side-by-side, and IRP reviews).
- Annual Mandate: Minimum 16 hours of in-house or division sponsored continuing education unit
CORE DUTIES & RESPONSIBILITIES
- Care Model: Deliver empathetic care aligning with the Comprehensive, Continuous, Integrated System of CARE.
- Team & Intake: Form multidisciplinary recovery relations (Psychiatrists, RNs, Peers). Execute face-to-face intake within 24 hours of crisis stabilization, 3 days of hospitalization, or 5 days of agency referral.
- Assessments & IRP: Complete integrated intake assessments, comprehensive evaluations, and consumer-informed Master Recovery Plans (IRP).
- Relapse Prevention: Build step-by-step Crisis Plans and Wellness Recovery Action Plans (WRAP) to counter eviction or higher-level care risks.
- 24/7 Availability: Provide 24/7 crisis service response; deploy face to-face if a consumer is in acute crisis. Manage field hours (7:30am–9:00pm) and on-call rotations.
- Clinical Consult: Immediately notify a QMHP for active psychosis,
suicidal, or homicidal ideation.
- Daily Living Support: Direct coaching in money management, cooking, cleaning, laundry, transportation, and medical access.
- Housing Transition: Assist with apartment hunting, landlord negotiations, furniture procurement, and living setup.
- Leisure & Social Skills: Help structure time and build social skills using role-playing, modeling, and assertiveness coaching.
- Transit & Scheduling: Secure attendance at all psychiatric and IRP appointments, providing transportation as needed.
- Contact Cadence: Face-to-face contact ≥1x monthly for stable consumers (≥6 months without ER/hospitalizations); daily/weekly for consumers with clinical changes or housing instability.
Pay: $24.00 - $32.00 per hour
