Community Care Physicians (CCP) is seeking a motivated and patient-focused Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing urology practice in Latham, NY. This is an exciting opportunity to work alongside experienced urologists and advanced practice providers in a collaborative, high-quality outpatient setting focused on comprehensive urologic care for adult patients.
Our practice provides advanced urologic services for both men and women, including treatment for kidney stones, urinary incontinence, BPH, overactive bladder, men's health conditions, and urologic cancers. Providers benefit from access to innovative technologies and minimally invasive treatment options, including in-office procedures, UroLift®, cystoscopy, Botox therapy, and lithotripsy services.
Evaluate, diagnose, and manage patients with a variety of urologic conditions in an outpatient setting
Perform comprehensive histories, physical examinations, and treatment planning
Order, interpret, and communicate diagnostic testing including laboratory and imaging studies
Provide patient education regarding diagnoses, medications, procedures, and treatment plans
Assist with management of acute and chronic urologic conditions including:
Kidney stones
Hematuria
Recurrent UTIs
Overactive bladder
BPH
Erectile dysfunction
Urinary incontinence
Perform office-based procedures and follow-up care as appropriate to training and experience
Collaborate closely with physicians, APPs, nursing staff, and administrative teams to ensure high-quality patient care
Maintain timely and accurate documentation within the electronic medical record (EMR)
Participate in quality improvement and patient satisfaction initiatives
Current New York State licensure as a Nurse Practitioner or Physician Assistant
DEA registration (or ability to obtain)
Graduate of an accredited NP or PA program
Strong clinical, communication, and organizational skills
Ability to work independently and collaboratively within a team-based environment
Prior urology, surgical, or specialty practice experience preferred, but new graduates with strong interest in urology are encouraged to apply
Experience with EMR systems
Full-time outpatient position
Monday–Friday schedule
No hospital call coverage and minimal office call responsibilities

Requirements:
- License and active registration as a physician assistant or registered nurse/nurse practitioner in New York State.
- Physician Assistant:
- Bachelor's or equivalent degree in physician assistant studies and successful completion of physician assistant training program approved by the NYS Department of Education or accredited by the Accreditation Review Commission on Education for the Physician Assistant, or a program that is determined by the Department to be the substantial equivalent of a licensure qualifying or accredited program.
- Attainment of passing grade on the Physician Assistant National Certifying Examination administered by the National Commission on Certification of Physician Assistants.
- Current certification by National Commission on Certification of Physician Assistants.
- At least one year of experience as a physician assistant in ambulatory health care.
- Current CPR certification.
- Nurse Practitioner:
- Successful completion of and graduation from a nurse practitioner education program acceptable to NYSED.
- Current certification as a nurse practitioner by the American Academy of Nurse Practitioners Certification Board (AANPCB) or the American Nurses Credentialing Center (ANCC).
- Successful completion of pharmacology coursework acceptable to NYSED.
- Current CPR certification.
