Responsibilities:

Monitor and report changes in resident condition to the physician and other healthcare providers
Coordinate with other healthcare professionals to ensure seamless care transitions
Maintain accurate and detailed records of resident care and treatment
Participate in quality improvement initiatives to enhance the overall quality of care
Collaborate with the therapy department to develop and implement comprehensive treatment plans
