Home Health Registered Nurse (Part-Time)
Location: Colorado Springs, Security Widefield, and Fountain, CO
Schedule: Part-Time l Weekly Pay l Two-Three days per week

Your Role:
As an Home Health RN, you will coordinate and provide skilled nursing care in patients’ homes. You will assess patient conditions, develop care plans, and collaborate with the care team to ensure high-quality outcomes. This role will be a combination of medical home care, such as case management of skilled nursing patients, and potentially oversight of non-medical programs. This job does have the potential to become a full-time position.
Responsibilities include:
- Perform comprehensive assessments (physical, psychosocial, environmental)
- Maintain detailed clinical documentation in compliance with home health regulations
- Communicate changes in patient status to physicians and the care team
- Coordinate care and services with auxiliary staff
- Take independent and appropriate action in home care situations
- Continuously work to improve patient outcomes and care quality
- Coordination medical and non-medical
Qualifications:
- Current Registered Nurse license in the state of Colorado & Multi-State
- CPR certification (current)
- Strong clinical judgment and communication skills
- Excellent time management and organizational skills
- Ability to work independently and manage a flexible schedule
- Valid driver’s license, auto insurance, and reliable transportation
- Must be available at least 2-3 days per week.
- Must meet all federal and state vaccination and health screening requirements (e.g., Influenza and TB)
- Must have 1 year experience as a Registered Nurse
