Job Overview
We are seeking a compassionate and dedicated Care Manager to join our team. The ideal candidate will coordinate and oversee client care plans, providing support and guidance to individuals facing behavioral health challenges and addiction issues. This role requires a strong understanding of social work principles, crisis intervention, and community resources to promote positive outcomes for clients. The Care Manager will work closely with families, healthcare providers, and community organizations to ensure comprehensive, person-centered care.
Responsibilities
- Develop and implement individualized care plans tailored to clients’ needs in areas such as behavioral health and addiction recovery.
- Provide crisis intervention and crisis management to de-escalate situations and ensure client safety.
- Conduct assessments related to social work, Section 8 housing, Medicare, and other social services programs.
- Offer motivational interviewing techniques to encourage behavioral change and engagement in treatment plans.
- Supervise and mentor staff or volunteers involved in client care activities.
- Coordinate with community agencies to facilitate access to resources like housing and food assistance.
- Monitor client progress and adjust care strategies accordingly while maintaining detailed documentation.
- Support clients through mentoring, counseling, or coaching sessions focused on personal development and stability.
Skills
- Strong background in crisis intervention and crisis management techniques.
- Experience in addiction counseling and behavioral health services.
- Knowledge of social work practices and Medicare/Medicaid regulations.
- Skilled in motivational interviewing to foster client engagement and motivation for change.
- Supervising experience with staff or volunteers in a social service setting.
- Ability to handle sensitive situations involving mental health issues with professionalism and empathy.
- Excellent communication skills for effective collaboration with clients, families, and community partners.

Pay: $50,219.79 - $60,479.74 per year
