POSITION SUMMARY
The Case Manager provides trauma-informed, survivor-centered case management services to individuals and families accessing residential shelter and outreach services. This role supports survivors in strengthening safety, achieving housing stability, and accessing community resources. The Case Manager partners with clients to develop individualized plans and move toward long-term safety and independence. This is an exempt professional position.
ESSENTIAL RESPONSIBILITIES
1. Residential and Outreach Client Case Management
- Provide case management support for residential and outreach clients.
- Meet with shelter residents within the first business day of arrival to begin a client-centered assessment of safety, needs, income, and housing planning, documented on the ISP.
- Schedule Child Development support as appropriate.
- Meet weekly (or more frequently as needed) with assigned residents for case planning and support.
- Support movement to safe, permanent housing in a timely manner.
- Provide aftercare follow-up, including collection and reporting of Needs Tracking Data at 2, 6, 12, 18, and 24 months.
- Maintain concurrent, accurate, and timely documentation of all client interactions.
- Provide aftercare case management and maintain linkage to community resources, including housing, employment, financial, and support services.
- Participate in weekly MDT and SHRT meetings and follow through on assigned action steps.
- Support client engagement in structured programming and assist with accountability when needed.
- Participate as an After-Hours Supervisor for 24/7 staff (rotating 1 week with other supervisors. Apx 6-8 weeks a year total).
2. Program Support and Services
- Facilitate individual and group educational services for residents.
- Support crisis hotline coverage as needed.
- Complete shelter intake and exit processes, including screening and documentation.
- Ensure compliance with agency policies and procedures.
- Complete assigned data collection and reporting accurately and on time.
- Perform other duties as assigned to support program operations.

PHYSICAL DEMANDS
While performing the duties of this job, the employee is typically required to be in a sedentary position or moving around an indoor environment. Must be able to move up and down stairs and accomplish tasks that require feeling, hearing, seeing, talking, reaching, bending, stooping, and light lifting work. Must be able to sit for extended periods of time.
WORK ENVIRONMENT
Work is primarily indoors with some outdoor activities and attendance at meetings in the community.

QUALIFICATIONS
Bachelor’s degree preferred in social work or related field or equivalent case management experience (2 – 4 years). LSW preferred. To perform this job successfully, an individual must be able to perform each essential responsibility satisfactorily. Pay: $20.00 - $24.00 per hour
