Position Summary
As a Home Health Registered Nurse, you will provide skilled nursing care to patients in their homes. You will complete comprehensive assessments, develop and implement individualized care plans, administer treatments, educate patients and caregivers, and collaborate with physicians and other healthcare professionals to ensure exceptional patient outcomes.
Responsibilities
- Conduct comprehensive patient assessments.
- Develop, implement, and update individualized care plans.
- Administer medications and treatments as ordered by the physician.
- Monitor patient progress and communicate changes in condition.
- Provide wound care and other skilled nursing interventions.
- Educate patients and family members on disease management, medications, and home care.
- Maintain accurate and timely clinical documentation.
- Coordinate care with physicians, therapists, and other members of the healthcare team.
- Ensure compliance with Medicare, state, and company policies.
Qualifications
- Active Florida Registered Nurse (RN) license.
- Current CPR/BLS certification.
- Minimum one year of nursing experience preferred.
- Home Health experience is an asset but not required.
- Excellent assessment, communication, and organizational skills.
- Valid Florida driver's license and reliable transportation.
- Ability to work independently while providing compassionate patient-centered care.

Preference will be given to candidates with experience in one or more of the following:
- Home Health
- Wound Care
- IV Therapy
- Chronic Disease Management
- Diabetes Education
- Trach Care
- G-Tube Care
