Social Worker
Job Summary:
We are seeking a compassionate and skilled Social Worker to join our team at Gulfport Nursing Center, a 121-bed skilled nursing facility located in beautiful South Pasadena, Florida, near the beaches. As a Social Worker, you will play a vital role in providing emotional and social support to our residents, families, and staff.
Responsibilities:
Assess the social and emotional needs of residents and develop individualized care plans to meet those needs
Provide support to residents and their families
Coordinate with interdisciplinary teams to ensure comprehensive care
Facilitate resident and family interactions, including communication and conflict resolution
Identify any issues or concerns of residents.
Maintain accurate and confidential records of resident interactions and progress
Participate in staff meetings and training programs to stay updated on best practices and policies
Collaborate with other departments to ensure seamless transitions and continuity of care
Requirements:
Bachelors degree in Social Work (BSW) or pyschology or related field
1 years of experience in social work or a related field as Social Worker
Excellent communication, interpersonal, and problem-solving skills
Ability to work effectively in a fast-paced environment and prioritize multiple tasks and responsibilities
