Role Description
Natural Speech & Language LLC is seeking a Licensed Physical Therapist (PT) to join our team as a Per Diem, 1099 Independent Contractor providing in-home physical therapy services to adult patients in Monroe, NJ and surrounding communities. This flexible position allows clinicians to accept referrals based on their availability and preferred service area.
The Physical Therapist will evaluate and treat adults with neurological disorders, Parkinson's disease, stroke, balance impairments, gait dysfunction, generalized weakness, orthopedic conditions, and other medically complex diagnoses. The PT will develop individualized treatment plans designed to improve strength, mobility, balance, endurance, safety, and functional independence while reducing fall risk and maximizing quality of life.
Services are provided in patients' homes, allowing clinicians to deliver personalized, patient-centered care in a familiar environment. The Physical Therapist will educate patients and caregivers on therapeutic exercises, home safety, fall prevention strategies, durable medical equipment, and home exercise programs to promote carryover between visits.
The PT will collaborate with physicians, speech-language pathologists, occupational therapists, behavioral health providers, caregivers, and other healthcare professionals to develop coordinated interdisciplinary treatment plans. Responsibilities include maintaining timely clinical documentation, participating in care coordination, and complying with Medicare, New Jersey state regulations, and professional practice standards.
Qualifications
- Current New Jersey Physical Therapist license in good standing.
- Medicare enrollment (or willingness to enroll and reassign benefits to the practice).
- Graduate of a CAPTE-accredited Physical Therapy program (DPT or equivalent).
- Minimum of one year of clinical experience preferred; home health, outpatient neurology, geriatrics, or neurological rehabilitation experience strongly preferred.
- Experience treating adults with Parkinson's disease, stroke, neurological disorders, balance impairments, and fall risk is preferred.
- Knowledge of evidence-based interventions for gait training, balance retraining, therapeutic exercise, functional mobility, and fall prevention.
- Excellent clinical assessment, communication, and documentation skills.
- Ability to work independently while collaborating effectively within an interdisciplinary healthcare team.
- Comfortable using electronic health records (EHR) for documentation.
- Current driver's license, reliable transportation, and willingness to travel for home visits within Monroe and surrounding areas.

Pay: $65.00 - $70.00 per hour
