We are seeking a dependable, compassionate, and caring Hospice Certified Home Health Aide (CHHA) – Per Diem to provide personal care and support to hospice patients in their homes and residential settings. The CHHA will work under the supervision of a Registered Nurse and follow each patient’s individualized plan of care.
The ideal candidate will be respectful, patient, attentive, and committed to preserving the comfort, dignity, and independence of patients receiving end-of-life care.
Duties
- Provide personal care to hospice patients according to the written plan of care and instructions provided by the supervising Registered Nurse.
- Assist patients with bathing, grooming, oral hygiene, dressing, toileting, and other activities of daily living.
- Assist with safe positioning, turning, transferring, walking, and range-of-motion exercises as assigned.
- Help maintain a clean, comfortable, and safe environment for the patient.
- Assist with changing bed linens and light housekeeping directly related to the patient’s care.
- Provide assistance with meals and feeding when included in the plan of care.
- Observe and promptly report changes in the patient’s physical condition, behavior, skin condition, comfort, or care needs to the supervising nurse.
- Provide companionship and emotional support to patients and their families.
- Follow infection-control procedures, safety precautions, and agency policies.
- Document all care provided accurately and promptly.
- Communicate professionally with patients, families, caregivers, nurses, and other members of the hospice team.
- Respect patient rights, preferences, privacy, cultural values, and confidentiality.
- Attend required training, competency evaluations, and in-service education.
- Perform only those duties assigned by the supervising nurse and permitted within the CHHA scope of practice.
Experience and Qualifications
- Current California Home Health Aide certificate required.
- Current CPR/BLS certification preferred.
- Hospice, home health, caregiving, or end-of-life care experience preferred.
- Compassionate and respectful approach to patients and families.
- Ability to follow a written plan of care and instructions from the supervising nurse.
- Strong communication, organization, and time-management skills.
- Ability to recognize and promptly report changes in a patient’s condition.
- Ability to complete accurate and timely visit documentation.
- Ability to safely assist patients with mobility and personal-care activities.
- Valid California driver’s license, reliable transportation, and current automobile insurance required.
- Ability to travel to patients’ homes throughout the assigned service area.
- Ability to successfully complete required background checks and health screenings.
