Comfort Keepers is seeking compassionate and dedicated PRN Home Health Registered Nurses to join our growing team. In this flexible role, you'll deliver personalized, high-quality nursing care directly in patients' homes, helping seniors and adults age gracefully, safely, and independently.

Key Responsibilities:
- Conduct comprehensive assessments of patient conditions and health needs.
- Develop and implement individualized care plans aligned with patient goals.
- Oversee paraprofessionals by providing training, supervision, and competency monitoring to ensure adherence to care plans, company policies, and state regulations.
- Coordinate effectively with physicians, therapists, caregivers, and interdisciplinary teams, promptly communicating changes in patient conditions and updating care plans as necessary.
- Administer prescribed treatments such as medication management, wound care, catheter changes, and other skilled nursing interventions.
- Educate patients and families on health management, home care procedures, and disease prevention strategies.
- Provide compassionate emotional support and guidance, ensuring patients and families have the resources needed for optimal care.
- Maintain accurate, timely documentation of patient care and clinical coordination activities.
Required Qualifications:
- Education: Graduate from an accredited nursing program (ADN or BSN).
- Licensure: Current, unrestricted RN license in Colorado or a participating compact state.
- Experience: Minimum of 2 years clinical nursing experience within the past 5 years, preferably in home health, hospice, or medical-surgical nursing.
- Certifications: Active BLS certification (additional specialty certifications highly valued).
- Skills: Strong clinical skills, comprehensive assessment abilities, care planning expertise, and confidence working independently in a home setting.
Additional Requirements:
- Valid driver's license, reliable transportation, and current auto insurance.
- Ability to pass criminal background checks and drug screenings.
- Experience with electronic health records (EHRs) preferred.
- Excellent communication skills with patients, families, and healthcare professionals.
- Strong organizational skills and ability to independently manage time effectively.
- PRN nurses are required to be available at least two days per week, including at least one 4-hour window between peak hours of 8AM-5PM
- Reliable transportation and ability to travel to client homes
