POSITION WILL PROVIDE COVERAGE FOR OUTPATIENT/PARTIAL HOSPITALIZATION SERVICES IN GASTON/LINCOLN/CLEVELAND COUNTIES
General Position Description:
This position will perform a variety of administrative and clinical tasks under the direction of the Nurse Manager. Under general supervision provides services to consumers participating in Phoenix’s medication clinics and is responsible for consumer flow. This may include general psychotropic medication clinics, Partial Hospitalization clinics, and/or any other service assigned by supervisor. Primary duties include supporting the medical and clinical services being provided to consumers, to include but not limited to obtaining medical information, history, and vitals from consumers; assisting consumers with basic needs providing liaison between medical and direct service/clinical staff; transportation; and other duties assigned by supervisor. Additional Primary duties for Partial hospitalization clinics may include obtaining lab work and transporting it to the contracted lab used by Phoenix. This may include administering SQ or IM injections under the direction of the ordering provider. This position will also be expected to enter physician orders and/or medication orders into ERX or electronic prescription service with accuracy as well as follow up with local pharmacies if needed.
Essential Job Duties:
1. Shall demonstrate the use of knowledge/skill/judgment and provide services that evidence ethical performance and meet the needs of persons served and other stakeholders. Performance tasks shall include, but not be limited to:
a. Linkage and coordination between outpatient services and medication clinics in order to provide consumers with effective medication services.
b. Under the direct supervision of the Outpatient Clinical Officer and RN Manager shall utilize knowledge/skill to facilitate med clinic services.
c. Acceptance of referrals, tracking clients’ fund sources, monitoring progress, and transition planning for those individuals being served in these programs.
d. Effective communication with consumers to enhance motivation and commitment to recovery process.
e. Taking accurate vital signs that include blood pressure, pulse, respirations, monitoring general health status, safety, observing self-administration of medications, medication administration, etc.
f. Completion of medical record documentation required for consumer participation in medication clinic.
g. Transport of consumers to medical facilities, as needed.
h. Use of a community-based team approach that utilizes the resources of the community, consumer, and treatment team, to achieve positive consumer outcomes, stabilization, and appropriate transition to other needed services and supports.
i. Shall demonstrate the ability to work/be a part of Phoenix’s organizational and therapeutic teams, and maintain positive working relationships with consumers, community stakeholders, referral organizations, and other employees/ departments of PCC.
j. Shall develop a working knowledge of applicable organizational policy and procedure and demonstrate compliance. This shall include, but not be limited to:
i. Personnel policies/procedures
ii. Employee Ethics/Corporate Compliance/Consumer Rights
(All Psych Tech’s/RSW’s/RMA’s/CMA’s are required to complete upon hire and annually thereafter mandatory professional ethics training)

n. Duties also include collection and review of patient accounts, authorization of client services and interfacing with fund sources, NC Tracks verification, identification of other payers with confirmation, entering administration information into the agency electronic medical records.
2. Clinical & Diagnostic Duties:
a. Lab Specimen Collection: Perform phlebotomy and collect required laboratory work from patients.
b. Specimen Logistics: Transport collected samples safely to the contracted Phoenix laboratory.
c. Medication Administration: Administer oral, subcutaneous (SQ) and intramuscular (IM) injections safely.
d. Provider Compliance: Deliver all injections strictly under the direction of the ordering provider.
e. Administrative & Pharmacy Coordination
f. Electronic Order Entry: Input physician and medication orders accurately into the ERX system.
g. Prescription Processing: Utilize electronic prescription services according to clinic protocols.
h. Pharmacy Liaison: Follow up with local pharmacies to resolve order issues or delays.
General Knowledge and Skills Required for Position:
General Knowledge of the dynamics of MH/DD/SA behavioral health disorders (BHD) and their impact upon persons served and their families; the bio/psycho/social factors involved in (BHD); up-to-date principles/techniques (evidence based/best practices) of crisis response, prevention, and intervention application to individuals possessing (BHD); the community resources available to persons with (BHD), to include behavioral health, social service, community services, and self-help groups; knowledge of billing and service record documentation requirements, NC Division of MH/DD/SA, ASAM/MCO, CARF and PCC policy/procedure.
General Skill in the application of crisis assessment/prevention/intervention techniques that are considered as ethical evidence based/best practices as defined by the NC Division of MH/DD/SA, and/or literature/research, and organizational policy/procedure; skill in the delivery of services using accepted crisis treatment modalities, to include, individual, family, marital, and in-community; skill in establishing/maintaining effective therapeutic and professional relationships with persons served and/or their family members; behavioral health providers; and, skill in accurately documenting services provided and billed.
Minimum Position Qualifications:
1. Valid LPN License
2. Preference will be given to applicants with behavioral health experience.

Pay: $28.85 - $31.25 per hour
