OBJECTIVE:
The Nurse-Family Partnership (NFP) nurse home visitor is responsible for providing comprehensive nursing services to women and their families who meet eligibility for the Nurse-Family Partnership Program (NFP Program). The Nurse Home Visitor is responsible for maintaining the highest level of standards in clinical nursing practice and adherence to the NFP model, NFP Visit-to-Visit Guidelines and standards of NFP and YDI policies and procedures.
MINIMUM QUALIFICATIONS:
•Associate degree or bachelor’s degree in nursing. Two (2) years’ experience in maternal/child, public health, home visitation or mental health/behavioral nursing preferred.
•Must possess current RN license (in good standing)
•Experience serving parents and children experiencing concentrated and overlapping barriers to accessing the care they need due to social, economic, and environmental disadvantages.
•Excellent written and verbal communication skills.
•Must have computer skills, including proficiency in data entry and documentation within data management systems or databases.
•Must be able to work flexible work schedules including some weekends and evenings.
•Must complete all required NFP initial education.
DUTIES & RESPONSIBILITIES (Not Inclusive):
•Provides home visits to women and their families eligible for NFP program in accordance with NFP model and NFP Visit-to-Visit Guidelines (eGuidelines).
•Adheres to nursing process and NFP model of home visitation.
•Carries a case load of up to 25 clients.
•Contributes to community engagement and outreach efforts, including occasional participation in Community Advisory Board meetings, representing NFP at community events through hosting informational booths, and fostering referral relationships with key partners such as WIC, OB clinics, CYFD, and other local organizations.
•Follows nursing process in implementing NFP into nursing practice.
•Develops therapeutic relationships, utilizes concepts of reflection and motivational interviewing with women and their families in a home visiting environment.
•Assesses physical, emotional, social and environmental strengths and risks of women and their families as they relate to the NFP domains.
•Assists women and their families in establishing goals and outcomes.
•Provides education, support and referral resources in assisting women and their families attaining their targeted goals.
•Consults and collaborates with other professionals involved in providing services to women and their families.
•Formulates nursing diagnosis based on nursing assessment and client goals.
•Evaluates client progress toward stated goals and NFP outcomes.
•Actively engages in professional development to meet all NFP nurse home visitor competency requirements.
•Meet with NFP nurse Supervisor weekly for clinical supervision.
•Attend information to support staff for timely and accurate data input to the designated data system.
•Participate in review and analysis of ETO reports for achievements and areas for improvement.
•Participate in quality improvement efforts.
•Represent the NFP vision, mission, and model un actions and verbally to both internal and external customers and colleagues.
•Performs other related duties as assigned.
JOB CHARACTERISTICS/PHYSICAL DEMANDS:
This position is exempt from overtime provisions of the Fair Labor Standards Act. This is a professional position which requires excellent organizational and communication skills, as well as judgment, tact and diplomacy in contacts with program clients, employees, and community representatives. Strict adherence is required to all regulations concerning confidentiality.
Background investigation to include fingerprinting will be required. Physical required at time of hire and may be required every five years. TB Test may be required. Travel may be required. Must submit valid driver’s license and proof of satisfactory car insurance under NM Motor Vehicle requirements.
