Responsibilities
- Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and personal hygiene to promote dignity and comfort.
- Support patients with mobility and transfer methods using equipment such as Hoyer lifts or patient transfer techniques to ensure safe movement.
- Monitor vital signs and observe patient conditions regularly to detect changes or concerns promptly.
- Prepare nutritious meals aligned with dietary requirements and assist with meal planning and preparation.
- Administer medication under supervision, follow care plans accurately, and document all care activities in compliance with HIPAA regulations.
- Provide companionship and emotional support, engaging clients in meaningful activities to enhance mental health and social engagement.
- Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning, laundry, and organizing personal spaces.
- Support clients during appointments or community outings by providing personal transportation assistance when needed.
Requirements
- Proven experience working with individuals with disabilities, neurodevelopmental disorders, dementia, Alzheimer’s, or in senior care settings such as nursing homes or assisted living facilities.
- Knowledge of patient observation techniques, medication administration, vital signs monitoring, and first aid/CPR certification.
- Familiarity with care plans, HIPAA compliance, manual handling procedures, and patient transfer methods including Hoyer lifts.
- Experience working with diverse populations including those with autism, intellectual disabilities, developmental disabilities, or in hospice care environments.
- Strong communication skills for client interaction and collaboration with healthcare teams or social workers.
- Ability to perform heavy lifting when necessary and assist with physical tasks safely while adhering to manual handling guidelines.
- Valid driver’s license or reliable transportation for home & community care support roles is preferred.
