Duties
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools like Hoyer lifts and transfer devices
- Monitor patient vital signs such as blood pressure, pulse, temperature, and respiration; accurately document medical information using Electronic Health Record (EHR) systems
- Medication reminders following prescribed protocols while maintaining strict adherence to HIPAA regulations for patient privacy and confidentiality
- Provide specialized care for individuals with dementia, Alzheimer’s disease, or other cognitive impairments through memory care techniques and patience-driven approaches
- Support patients with catheter care, wound management, hospice needs, and other complex medical procedures under supervision or as trained
- Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe environment tailored to each patient’s needs
- Document all patient interactions thoroughly in medical records and communicate effectively with healthcare teams regarding changes in condition or concerns
Experience
- Prior experience working in assisted living facilities, nursing homes, or home health environments is highly preferred
- Demonstrated ability to observe patient conditions closely and respond promptly to changes in health status
- Familiarity with EMR/EHR systems for accurate documentation of patient care activities
- Hands-on experience assisting with ADLs, medication administration, and medical procedures such as catheter care or vital sign monitoring
- Knowledge of medical terminology relevant to long-term care, dementia, hospice, and complex medical conditions
- Certification in CPR is required; additional certifications such as hospice care training or dementia care are a plus
- Experience working with individuals with developmental disabilities or disabilities is highly valued
