Physical Therapist (PT)
We are seeking a licensed and dedicated Physical Therapist to join our interdisciplinary team. As a Physical Therapist at Countryside, you will be responsible for designing and implementing individualized treatment plans to improve the functional abilities of our patients. Your expertise will help patients overcome physical disabilities, injuries, or illnesses, and restore their ability to live independently.
Physical Therapist (PT) Responsibilities:
Conduct comprehensive evaluations to assess patients' physical limitations and develop personalized treatment plans
Implement and modify treatment plans as needed to achieve patients' goals and objectives
Provide one-on-one physical therapy sessions, group therapy, and education to patients and their families
Collaborate with other healthcare professionals to ensure effective communication and coordination of care
Maintain accurate and timely documentation of patient progress and treatment plans

Physical Therapist (PT) Requirements:
Master's degree in Physical Therapy (PT) from an accredited program
Current licensure to practice as a Physical Therapist in the state of Tennessee
Strong communication and interpersonal skills
