We are seeking a motivated and compassionate Physical Therapist (PT) to join our skilled nursing facility rehabilitation team. The Physical Therapist will evaluate residents, develop individualized treatment plans, and provide therapeutic interventions to improve strength, mobility, balance, safety, and functional independence.
Responsibilities
Perform comprehensive physical therapy evaluations and assessments
Develop individualized treatment plans based on resident goals and clinical needs
Provide therapeutic exercises, gait training, transfer training, balance activities, and mobility interventions
Monitor resident progress and modify treatment plans as appropriate
Accurately document evaluations, treatment sessions, progress notes, and discharge summaries in compliance with regulatory standards
Collaborate with physicians, nursing staff, occupational therapists, speech therapists, and interdisciplinary care teams
Educate residents, families, and caregivers regarding therapy programs, safety, and functional mobility
Ensure compliance with facility policies, state regulations, and professional standards of practice
Maintain a safe and supportive rehabilitation environment
Qualifications
Doctorate or Master’s degree from an accredited Physical Therapy program
Current state licensure as a Physical Therapist (PT)
Strong clinical assessment and treatment planning skills
Excellent communication and interpersonal abilities
Ability to work independently and collaboratively within a multidisciplinary team
