Job Overview
We are seeking a dedicated and energetic Licensed Vocational Nurse (LVN) to join our team as a Field Nurse, providing vital in-home and community-based nursing care. Duties
- Conduct comprehensive patient assessments including vital signs, physical examinations, and physiological evaluations to determine individual care needs.
- Administer medications, injections, IV infusions, and perform catheterizations following strict medical protocols and HIPAA regulations.
- Provide specialized care for patients with complex conditions such as dementia, Alzheimer's disease, traumatic injuries from Level I trauma centers, or those requiring hospice or palliative care.
- Manage medical documentation accurately using EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems like Epic, Cerner, Athenahealth, or eClinicalWorks.
- Assist with diagnostic evaluations including specimen collection/processing and medical imaging procedures while maintaining aseptic techniques.
- Educate patients and their families on health coaching topics such as nutrition, medication management, discharge planning, and chronic disease management including pain control and infection prevention.
- Collaborate with interdisciplinary teams to coordinate case management plans for patients in inpatient settings like ICU or PICU or outpatient environments such as clinics or home & community care.
- Ensure compliance with all safety standards including sterile processing, infection control practices, and proper use of medical equipment like ventilators or spinal taps.
Experience
- Valid LVN license with current state licensure required; additional certifications in Basic Life Support (BLS) preferred.
- Proven experience working as a nurse in diverse environments such as hospitals (including ICU and trauma centers), nursing homes, assisted living facilities, or home health settings.
- Critical care experience especially within Level I or Level II trauma centers is highly desirable.
- Demonstrated proficiency with EMR/EHR systems such as Epic or Cerner; familiarity with ICD coding (ICD-9/ICD-10) is a plus.
- Background in managing complex patient cases involving acute pain management, infectious disease care, or post-acute recovery processes.
- Experience working with individuals with disabilities or developmental challenges including dementia and Alzheimer’s care is advantageous.
- Knowledge of anatomy, physiology, pharmacology related to medication administration and tube feeding procedures.
