Responsibilities:
1. Resolves drug distribution problems assuring accurate and timely dose delivery to all patients
2. Interprets Clinical and Pharmacokinetic Lab Data in order to formulate medication monitoring plans
3. Problem Identification and Resolution
3.1 Communicate and documents problem detection, relevant findings, recommmendations,and necessary follow-up
4. Provides drug information to nurses, prescribers, and other healthcare professionals
5. Educational Activities
6. Documents medication use variances (MUVs) and Adverse Drug Reactions (ADR's)
7. Complete department education and competancies in a timely manner in order to satisfy regulatory requirements
Qualifications:
EDUCATION
Graduated from an accredited College of Pharmacy with a B.S. or Pharm. D. degree. Completion of a residency in hospital pharmacy from a program accredited by the American Society of Hospital Pharmacists is desirable but not required.
EXPERIENCE
Two years experience in an oncology based hospital or outpatient oncology pharmacy setting is preferred for this position
LICENSURE
Licensed to practice pharmacy in Connecticut or eligible for reciprocation
SPECIAL SKILLS
This individual should demonstrate knowledge of oncology pharmacology and therapeutics, and understand pharmacy systems of drug distribution, manufacturing technology, clinical trials when applicable, and formulary management. Excellent communication skills and the ability to work with others required.
