Care Coordinator (Full Time)
FirstLight Home Care is seeking candidates for the position of Full-Time Care Coordinator. The Care Coordinator is responsible for the coordination of care of the clients. This individual directly interfaces with clients performing intakes, quality evaluations, originating service plans, scheduling and supervising care. Participating in the on-call rotation is required.
Responsibilities Include :
- Enters, maintains, and corrects client data into the computer.
- Visits prospective clients/clients after referrals are made.
- Performs initial and ongoing in-home evaluation, caregiver introductions and supervision of services.
- Completes appropriate visit records in a timely manner.
- Handles coordination of care for client services.
- Reports changes in client condition to agency employees involved in client’s care.
- ON-CALL ROTATION REQUIRED- Takes calls after hours and weekends to handle emergencies, scheduling issues, and client referrals and intakes.
- Participate in the quality assurance reviews and evaluations of the agency’s services.
Qualifications:
- LPN or STNA preferred
- Experience in home care or knowledge of the healthcare industry preferred.
- Prior scheduling experience preferred.
- Requires proficiency in computer skills.
- Prior on-call experience preferred.
