Responsibilities
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support.
- Monitor patient health by observing vital signs, reporting changes, and maintaining accurate medical documentation using Electronic Health Record (EHR) systems.
- Provide medication reminders and assist with medication administration following prescribed instructions and home healthcare regulations.
- Support patients with specialized care needs such as Alzheimer’s disease, dementia, memory care, and end-of-life care with patience and compassion.
- Operate patient transfer methods safely using equipment like Hoyer lifts or transfer belts while adhering to proper patient transfer techniques.
- Maintain patient confidentiality and adhere to HIPAA guidelines while managing sensitive medical records and documentation.
- Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe living environment for clients.
- Communicate effectively with patients and their families to address concerns, provide updates on care plans, and ensure client satisfaction.
- Follow all safety protocols including first aid, CPR certification requirements, and basic life support procedures to respond promptly during emergencies.
Requirements
- Proven experience working as a Home Health Aide or in assisted living facilities; experience in nursing homes or hospice care is highly valued.
- Strong knowledge of medical terminology, catheter care, medication management, and vital signs monitoring.
- Demonstrated ability to work with individuals with disabilities or developmental disabilities; experience with geriatrics or dementia care is preferred.
- Familiarity with EMR/EHR systems for documentation; understanding of home health care regulations is essential.
- Certification in first aid and CPR; willingness to obtain additional certifications as required by state or agency standards.
- Ability to lift heavy objects safely and perform physically demanding tasks associated with patient transfer and mobility assistance.
- Excellent communication skills for effective client interaction and team collaboration; compassionate caregiving attitude required.
- Valid driver’s license and reliable transportation may be necessary for travel between client locations.
