About the Program:
The Connecticut Adult Family Living (AFL) program is a service provided through the Connecticut Home Care Program for Elders (CHCPE). It allows individuals who are 65 or older to live in a private home with a principal caregiver who provides daily personal care and supportive services. The program is designed to help individuals remain in a safe, home-based environment and avoid unnecessary institutionalization.
Position Summary:
We are seeking a compassionate and skilled Registered Nurse (RN) or Licensed Practical Nurse (LPN) to conduct client assessments and provide education for our Adult Family Living program in Connecticut. The ideal candidate will have a strong clinical background, excellent communication skills, and a passion for helping clients and their family caregivers succeed in the home setting. This role is crucial for ensuring the health, safety, and well-being of our clients and for facilitating a positive and supportive home care environment.
Key Responsibilities:
- Client Assessments:
- Conduct comprehensive in-home assessments for new and existing AFL clients to develop individualized care plans and determine their functional, medical and psychological needs.
- Utilize standardized assessment tools to evaluate the client's physical and mental health, ADL capabilities, home environment, and safety risks (e.g., fall risks).
- Collaborate with the care team to develop and revise individualized plans of care that address the client's needs and promote their independence.
- Perform regular reassessments to monitor changes in the client's condition and adjust the care plan as needed.
- Education and Training:
- Educate clients and their family caregivers on all aspects of the client's home care needs, including medication management, vital sign monitoring, and proper use of medical equipment.
- Provide training and ongoing support to the family caregiver on essential personal care skills, such as bathing, dressing, and transferring.
- Teach caregivers how to recognize and report changes in the client's health status and how to respond to emergencies.
- Provide resources and information to the client and caregiver to help them navigate the healthcare system and connect with community support services.
- Documentation and Communication:
- Maintain accurate and timely documentation of all assessments, care plans, and client interactions in accordance with program standards and state regulations.
- Communicate effectively with the client, caregiver, physicians, and other members of the interdisciplinary team to ensure coordinated and holistic care.
- Provide regular feedback and reports to the program director regarding client needs and assessment outcomes.
- Serve as a liaison between the client, caregiver, and other healthcare providers to advocate for the client's needs.
Qualifications:
- Active and unrestricted RN or LPN license in the State of Connecticut.
- Proven experience in home care, geriatric nursing, or a related field.
- Strong skills in patient assessment and the ability to work independently are essential for success in this role.
- Excellent interpersonal and communication skills, with the ability to build rapport with clients and their families.
- Familiarity with the Connecticut Home Care Program for Elders (CHCPE) and the Adult Family Living program is highly desirable.
- Ability to work independently and manage time effectively.
- Proficiency in electronic health records (EHR) and basic computer skills.
- Valid driver's license and reliable transportation for in-home visits.
