Duties
- Conduct comprehensive patient assessments to determine functional limitations and develop effective treatment plans tailored to individual needs.
- Implement evidence-based physiotherapy interventions, including manual therapy, trigger point therapy, and therapeutic exercises to promote recovery.
- Collaborate closely with healthcare professionals such as physicians, nurses, and occupational therapists to coordinate patient care and optimize treatment outcomes.
- Manage discharge planning processes by preparing detailed care plans for patients transitioning from inpatient to outpatient or home health settings.
- Maintain accurate clinical documentation in compliance with healthcare standards, including progress notes, treatment records, and therapy reports within electronic health records (EHR) systems.
- Provide education and guidance to patients and their families on rehabilitation techniques, injury prevention, and health maintenance strategies.
- Participate in outpatient rehabilitation center activities, inpatient hospital teams, or home & community care programs to deliver consistent and high-quality patient care.
Skills
- Strong knowledge of physiology, anatomy, orthopedics, neurology, pediatrics, geriatrics, and related medical terminology.
- Proven experience in outpatient clinic environments, inpatient hospital settings, or home health services with a focus on patient rehabilitation.
- Expertise in developing treatment plans that incorporate discharge planning and tailored interventions for diverse populations including geriatric and pediatric patients.
- Proficiency in managing electronic health records (EHR) and electronic medical records (EMR) systems while adhering to clinical documentation standards.
- Excellent client communication skills to effectively educate patients about their treatment processes and foster positive therapeutic relationships.
- Ability to work collaboratively with multidisciplinary teams to enhance patient outcomes through integrated care approaches.
Pay: $40.00 - $43.00 per hour
