Duties
- Assist residents with Activities of Daily Living (ADLs), including personal hygiene, grooming, dressing, and mobility support using manual handling techniques such as Hoyer lifts and patient transfer methods.
- Monitor patients’ vital signs, observe health status, and report changes to healthcare professionals to ensure optimal care.
- Prepare nutritious meals and assist with meal planning in accordance with dietary needs and care plans.
- Support medication administration following HIPAA regulations and established protocols to ensure safety and compliance.
- Provide patient care during home health visits or in community settings, including Alzheimer's care, dementia care, and memory support for individuals with neurodevelopmental disorders or developmental disabilities.
- Maintain detailed documentation of patient progress, care plans, and incident reports while ensuring confidentiality and adherence to HIPAA standards.
- Facilitate transportation for appointments or community activities, providing safe personal transportation assistance as needed.
Experience
- Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly desirable.
- Demonstrated ability to perform patient observation, monitor vital signs, and assist with medication administration.
- Knowledge of specialized care such as Alzheimer's disease, dementia care, hospice care, or autism support enhances your effectiveness in this role.
- Experience working with individuals with disabilities—including intellectual disabilities or neurodevelopmental disorders—will be highly valued.
- Familiarity with patient transfer methods like Hoyer lifts and patient personal care assistance ensures safe handling practices.
- Strong communication skills for client interaction and team collaboration are essential for success.
- Certification in CPR and First Aid is required; ongoing training in manual handling and heavy lifting techniques is beneficial.
