JOB DESCRIPTION: VAN DRIVER

Reports To: Administrator
GENERAL PURPOSE
To provide transportation for residents to and from facility in accordance with current federal, state and local standards governing the facility, to ensure that the highest degree of quality of care is maintained at all times.
ESSENTIAL JOB DUTIES
Transport residents to and from facility. Maintain complete and accurate logs.
Operate vehicle in accordance with applicable traffic laws, company Vehicle Policy, state and federal regulations,
Follow guidelines for vehicle maintenance and reporting vehicle malfunctions, traffic accidents, etc. as describe in Vehicle Policy.
Maintain cleanliness of vehicle Maintain proper forms in vehicle
Understand, comply with and promote all rules regarding rights, promote positive relationships with residents, visitors, and regulators, to include a professional appearance and attitude
Immediately report any hazardous conditions, equipment, accidents and/or incidents
Attend and participate in training, education activities and staff meetings, assist in orientation and training other staff
Monitor resident condition while transporting to and from appointment CNA will obtain code status of resident he/she is transporting
If an emergency situation arise, CNA is to call 911 and inform DON and Administrator
DRIVERS ARE NOT TO USE ANY TYPE OF CELLULAR DEVICE WHEN OPERATING THE FACILITY VEHICLE. THIS INCLUDES ANY TYPE AIR BUDS, HEADPHONES, ETC. IF YOU FIND IT NECESSARYTO CALL FACILITY OR FOR ANY OTHER EMERGENCY, PLEASE PULL OVER.
DRIVERS SHALL NOT USE THE. VEHICLE'S RADIO NOR ANY OTHE MEDIA DURING THE OPEFIATON OF THE VEHICLE.
Will assist on floor as a CNA when notouton transportation
Any other duties assigned by Director of Nursing or Administrator
QUALIFICATIONS
- Must have a valid and current Driver's License with no driving violations on record
- Must have a high school diploma or equivalent
- Must have ability to understand and apply training, and in-service education
- Must be capable of maintain regular attendance
- CPR Certified
KNOWLEDGE, SKILLS, ABILITES, AND BEHAVIORS
Must agree to abide by the facility Vehicle Policy
Must have ability to read, write, and follow oral and written directions at a level necessary to accomplish the job
Must be able to maintain confidentiality regarding resident and company proprietary information
Must be able to relate positively and favorably to residents, families and other employees at all levels
PHYSICAL AND SENSORY REQUIREMENTS
Mobility, reaching, bending, lifting, grasping, fingering, fine had coordination, ability to talk and hear; ability to read and write, ability to communicate with personnel, ability remain calm under stress, ability to adequately lift, support and bodily manipulate the weight of a residents.
