Duties
- Perform comprehensive patient assessments to evaluate physical, emotional, and psychological needs, ensuring personalized care plans are implemented effectively.
- Administer medications, including IV infusions and injections, following strict protocols to manage pain and symptoms such as acute pain or dementia-related behaviors.
- Monitor vital signs, conduct physical examinations, and document medical records accurately using EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems like Epic or Cerner.
- Collaborate with interdisciplinary teams to coordinate care, discharge planning, and case management strategies that support patients' comfort and safety.
- Provide education to patients and families about disease processes such as Alzheimer's disease, dementia care, or infectious disease management to promote understanding and engagement.
- Assist with tube feeding, catheterization, wound care including suturing or sterile processing techniques, and other complex nursing procedures within hospice guidelines.
- Maintain compliance with HIPAA regulations while ensuring the highest standards of infection control and patient confidentiality are upheld at all times.
Skills
- Proven experience in hospice care
- Strong knowledge of medical terminology, physiology, anatomy
- Skilled in patient assessment, medication administration—including injections—and IV insertion with aseptic technique.
- Familiarity with EMR/EHR systems like Epic, Athenahealth, or eClinicalWorks for accurate documentation.
- Ability to perform physical examinations and vital sign monitoring efficiently while demonstrating excellent communication skills.
- Experience working with diverse populations including individuals with developmental disabilities or those requiring memory care or behavioral health support.
- Capable of managing complex cases involving pain management, ventilator use, or gastrostomy tube feeding in home or inpatient settings.
