St. Andrew’s Community Resource Center (CRC), a ministry of St. Andrew’s Lutheran Church in Mahtomedi, Minnesota, seeks to transform lives through Christ’s love by equipping families and individuals with resources that lead to housing and employment stability.
We are currently seeking a dependable and caring Case Manager. The Case Manager supports and affirms the identity, mission and vision of St. Andrew’s Lutheran Church by providing support and resources for individuals and families in need, helping families find resources, and providing case management services to guests.
Primary Responsibilities:
- Manage a case load of 10-12 families at a time who are receiving services as part of our sheltering program (St. Andrew’s Family Shelter) or supportive housing program (Passages).
- Orientate new families into programs.
- Create goal plans with families and evaluates their progress and accountability.
- Build rapport with collaborating organizations to help find solutions to housing barriers.
- Works on transitional housing plans for families exiting our sheltering program into stable housing.
- Provide case management for "walk-in" guests.
- Process rental assistance for families.
- Maintain relationships with partner agencies in of eviction prevention.
- Collect and file necessary documentation including a signed release and form of identification.
- Document case management notes of family status into software program.
- Assist CRC with compliance of social service standards of homelessness.
- Remain up-to-date regarding knowledge of state rules and regulations relating to social services and serving the homeless population.
- Collaborate with Intake Coordinator regarding families calling for potential shelter.
- Conduct intake interviews with families for sheltering program and reviews background checks.
- Evaluate perspective families for sheltering program based on their needs, barriers, and goals.
- Research and maintain an up-to-date list of referral agencies and collaborating partners for the success of our families.
- Other duties as assigned to meet the needs of the organization.
Minimum Qualifications:
- High School Diploma/GED
- 3+ years of case management experience with a non-profit organization providing similar services

Preferred Qualifications:
- Bachelor’s degree in social work or related field
- LSW, LCSW, LMSW, or closely related licensure
- Bilingual

- Ability and comfort to discuss matters of faith with peers, supervisor, families, volunteers, and other church staff
- Working knowledge of the causes and impacts of living in poverty
- Skill in recognizing and maintaining confidential information
- Skill in listening and conveying understanding of guests’ problems or issues to find resolution
- Skill in expressing empathy for others
- Skill in working with a wide variety of individuals of all socio-economic levels
- Oral and written communication skills sufficient to exchange information effectively and professionally with coworkers, supervisor, and guests
- Skill in working as part of a team and collaborating with colleagues
- Skill in prioritizing assignments to complete work in a timely manner
- Knowledge of available resources in Ramsey and Washington counties
- Working knowledge of diverse economic and cultural populations
- Skill in problem solving and resource research
- Proficiency in the use of a computer; including internet, email, Microsoft Office, database and software systems, as well as proficiency in using equipment found in a typical office environment
Work Hours:
- The specific work schedule will be determined in collaboration with Supervisor. Typical schedule will be Monday-Friday, 40 hours per week with some evening and weekend availability required for special events.
- This position is required to participate in Hospitality/Welcome Desk shifts on Christmas Eve and Easter as well as some churchwide “All Hands on Deck” special events.
