Job Description
POSITION DESCRIPTION/ PRIMARY PURPOSE
The Aazhoomon Care Coordinator provides compassionate, culturally responsive support to individuals and families by walking alongside members on their healing journeys. This role emphasizes relationship-building, advocacy, and consistent follow-through to help members identify strengths, overcome barriers, and access supportive services.
The Aazhoomon Care Coordinator works collaboratively across programs and with community partners to support member healing and recovery efforts. Care is delivered utilizing blended western and indigenous approaches that honoring Ojibwe values and the Seven Grandfather Teachings. This position requires humility, empathy, and a strong desire to make a meaningful difference in the lives of others.
DUTIES AND RESPONSIBILITIES
- Provide one-on-one support to members in a respectful, compassionate, and culturally grounded manner
- Walk alongside members in their healing journeys by encouraging selfdetermination, resilience, and personal growth
- Assist members in identifying goals, strengths, and barriers, and support followthrough on action steps
- Identify supportive community resources and advocate for member needs with community partners
- Maintain member files and grant reporting, documenting member interactions and progress
- Collaborate with team members and community partners to ensure coordinated services and holistic support
- Participate in meetings, trainings, and community engagement activities including public speaking as required
- Model respectful behavior rooted in Ojibwe values, including honesty, humility, bravery, wisdom, love, respect, and truth
- Perform other duties as assigned
SKILLS/ KNOWLEDGE/ REQUIREMENTS
- Post Secondary Education in Human Services, Social Work, Community Health or related field
- Experience in supportive services, community work, or related fields is preferred
- Strong commitment to compassionate, person-centered care and a desire to make a difference
- Ability to work with humility, empathy, and respect for diverse lived experiences and backgrounds
- Knowledge of, or willingness to learn and practice, Ojibwe values and approaches in care delivery
- Excellent communication skills, including listening, verbal, and written abilities
- Ability to build relationships and work effectively with a wide variety of people
- Strong follow-through and organizational skills; able to complete tasks on time and meet expectations
- Demonstrated problem-solving skills and ability to navigate challenges effectively
- Dependable, punctual, and able to follow directions and guidance
- Ability to work independently and as part of a team
- Ability to maintain confidentiality and professionalism at all times
- Computer skills and ability to complete required documentation
- Build trusting relationships through active listening, empathy, and consistent communication
- Demonstrate dependability by following directions, completing tasks, and maintaining accountability
- Maintain confidentiality and professional boundaries at all times
- Identify challenges or barriers and use problem-solving skills to create solutions and support member success

Salary: $23.00 per hour-Part-time, Temporary
