Position Summary
The Care Coordinator/Data Specialist is a bachelor's-level professional responsible for supporting CCRT operations through care coordination, data management, documentation oversight, quality assurance, and performance monitoring. Under the supervision of the Clinical Care Coordinator, this position supports referral coordination, service linkage, participant follow-up, and program reporting while collaborating with the multidisciplinary team to promote continuity of care and engagement in services for up to 90 days following a crisis response.
This position serves as the primary coordinator for CCRT program data collection, tracking, reporting, and supporting continuous quality improvement efforts, while assisting with care coordination activities across the multidisciplinary team.
Duties and Responsibilities

Coordinate referrals and facilitate warm handoffs to behavioral health, medical, housing, and community-based services.

Conduct outreach and monitor participant engagement through 30-, 60-, and 90-day follow-up to support continuity of care and recovery.

Maintain accurate documentation and participant records in accordance with agency and regulatory requirements.

Collaborate with clinicians, counselors, Peer Recovery Support Specialists, and community providers to coordinate services and address participant needs.

Monitor and track program data, including referrals, follow-up activities, service engagement, and program outcomes.

Prepare routine reports and assist with data collection, quality assurance, and performance improvement initiatives.

Assist with maintaining community resource information and supporting efficient program operations.

Provide administrative support for audits, site visits, accreditation reviews, and other program compliance activities.

Participate in multidisciplinary team meetings, case reviews, and ongoing program planning.

Promote trauma-informed, person-centered, and recovery-oriented practices in all interactions.

Perform other duties as assigned to support the mission and goals of the Community Crisis Response Team.
Requirements
Education:
-
Bachelor's Degree in Social Work, Psychology, Human Services, Public Health or related field required.
Experience:
-
Minimum of two (2) years of experience in behavioral health, crisis services, care coordination, case management, quality improvement, or community-based human services preferred.

Experience working with individuals experiencing mental illness, substance use disorders, homelessness, or co-occurring disorders preferred.

Experience using Electronic Health Record (EHR) systems and computer applications (including Microsoft Office and Google Workspace) for documentation, data management, reporting, and quality improvement activities; experience developing spreadsheets, dashboards, and performance reports is strongly preferred.
Knowledge, Skills & Competencies

Knowledge of behavioral health systems of care, community-based resources, care coordination and case management practices, trauma-informed care, recovery-oriented systems of care, HIPAA and confidentiality regulations, documentation standards, and quality assurance practices.

Excellent verbal and written communication, documentation, organizational, and time management skills.

Ability to prioritize tasks, exercise sound judgment, and make effective decisions in fast-paced, high-pressure environments.

Strong interpersonal skills with the ability to build collaborative relationships and effectively navigate community resources and service systems.

Proficiency in data management, electronic health record (EHR) systems, and computer applications to support documentation, reporting, and program operations.
