The Case Manager will oversee and effective operation of the service provided by the individual support and community integration. The Case Manager will assist clients in the community and be able to advocate for them when needed. The Case Manager is a resource to clients living independently and works cooperatively with agencies and other community members to facilitate program goal achievement of supporting all aspects of the individual.
The Schedule Hours for this position : M-F 9AM-5PM
Additional Job Duties:
Maintain positive work relationship with CAJ Services employees, individuals, families, referral agencies, and other community members to ensure satisfaction with program operations.
Formally and informally respond to needs, desires and interests of clients and program participants by conducting home visits and having regular communication with clients on assigned caseload.
Conduct home visits with Clients to assess needs and to develop support plans.
Complete all necessary paperwork related to the individual support, Companion and other group services.
Provide motivation of program participants with community needs during support programs as requested by the Program Director. Community needs might including, completing doctor's appointment, companionship, scheduling community visits and exploring the community.
The Case manager will actively looking for activities in the community that will help clients build relationship in their communities.
