Myrtle Health & Physiatry is expanding in the Lexington market and seeking a motivated Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing clinical team. This role focuses on delivering high-impact physiatry and pain management care within the skilled nursing facility (SNF) setting.
What You’ll Do
- Provide on-site care at designated skilled nursing facilities
- Evaluate develop care plans, consult on rehab, manage patients with pain, spasticity, and functional limitations
- Develop individualized treatment plans focused on mobility and recovery
- Treat patients recovering from stroke, surgery, and complex medical conditions
- Collaborate closely with therapy teams (PT/OT/ST) to drive functional progress
- Coordinate care with interdisciplinary teams to improve outcomes and reduce rehospitalizations
What We’re Looking For
- Active NP or PA license in Virginia (required)
- Experience in physiatry, pain management, geriatrics, or post-acute care preferred
- Ability to work independently with strong clinical judgment
- Excellent communication skills and a collaborative mindset
Position Details
Employment Options:

Compensation:
- 1099 Starting at $75/hour+ (flexible based on experience)
- Part-Time: $3,500 – $12,000+ per month
- Full-Time: $105,00 – $170,000 annually + benefits
Schedule:
- Flexible 8-hour shifts (typically 9:00 AM – 4:00 PM)
- Weekdays only — no evenings or weekends required (optional if preferred)
