Job Overview
We are seeking a dynamic RN/BSN Home Care Intake Coordinator to join our dedicated team. In this vital role, you will be the first point of contact for new clients and their families, coordinating intake assessments and ensuring seamless onboarding for home care services. Your expertise in nursing, patient assessment, and healthcare coordination will help us deliver personalized, high-quality care that promotes independence, healing, and dignity.
Duties
- Work with Director of Nursing/Administrator
- Help prospective clients and family members understand our services
- Enter new prospects and client information into staffing software
- Coordinate new client intake and perform new client assessments
- Help clients with questions about care, schedules, and billing
- Create and update care plans
- Document all significant client and caregiver interactions
- Clearly communicate care plans to caregivers
- Work with HR department and assist with new caregiver orientations
- Assist Administrator and other departments will all aspects of business operations
- Assist with client scheduling and caregiver staffing as needed
- Maintain relationships and work with referral sources
Requirements
- A RN/BSN degree and a valid RN license
- At least 5 years of Geriatric Nursing experience
- Passion for helping seniors and their families
- Caring, compassionate, and highly motivated
- Well-developed customer services kills
- Represent the organization in a positive and professional manner
- Excellent verbal and written communication skills
- Proficient in the use of MS Word, Excel, Outlook, and EMR experience
- Critical care experience including ICU, emergency medicine, or trauma center settings preferred

Pay: From $85,000.00 per year
