Having many sites per day/visit or points per week: 30 points per week

Point of Care/HomeCare HomeBase Required
Oasis Required
Home Health experience a must!
Counties Covering: Baltimore City and Baltimore County, possible AA County and Howard County if patients are not enough.
Division: Home Health
Is CPR/BLS, Valid Driver Licenses and Car Insurance required: Yes
Shift- M-F 8am-5pm, Possibly on-call but not requiring depends on business needs
Any certifications/licenses required?: RN License in state of Maryland or compact license
Responsibilities
- Conduct comprehensive assessments and develop individualized care plans for high-risk members.
- Coordinate care with physicians, social workers, discharge planners, and specialty resources.
- Arrange appropriate, cost-effective care levels and non-medical support such as housing and transportation.
- Monitor clinical, functional, and financial outcomes while promoting health and preventing adverse events.
