Salary: $65,000–$75,000 per year (DOE)

Coordinate and oversee the facility's social services program

Complete psychosocial assessments and develop individualized care plans

Assist residents and families with discharge planning and community resources

Advocate for resident rights and promote quality of life

Provide emotional support and crisis intervention for residents and families

Work closely with the interdisciplinary team to ensure residents receive comprehensive care

Maintain accurate documentation in accordance with state and federal regulations

Help ensure compliance with all applicable healthcare regulations

Bachelor's degree in Social Work or a related human services field preferred

Previous experience in a skilled nursing or healthcare setting preferred

Knowledge of Medicare, Medicaid, and long-term care regulations

Excellent communication, organizational, and interpersonal skills

Ability to work independently while collaborating with a multidisciplinary team
