Responsibilities
Conduct comprehensive patient evaluations in home settings and develop individualized PT treatment plans
Deliver therapeutic exercises, manual therapy, and functional mobility training to improve strength, balance, and independence
Educate patients and caregivers on safety, home‑exercise programs, and long‑term self‑management strategies
Coordinate care with nurses, OTs, social workers, and physicians to ensure seamless, interdisciplinary support
Document patient progress, outcomes, and plan updates in the electronic medical record within required timeframes
Maintain HIPAA compliance, safety standards, and equipment sanitation on every visit
Qualifications
Active California Physical Therapist license (Doctorate or Master’s preferred)
Open to new grads
Current CPR/BLS certification
Reliable transportation with auto insurance and valid CA driver’s license
Strong clinical reasoning, communication, and patient‑education skills
Ability to work independently, manage a dynamic caseload, and adapt to changing patient needs
