Social Worker
Fairview Skilled Rehab & Nursing Home – Queens, NY
Fairview Skilled Rehab & Nursing Home is seeking a compassionate and organized Social Worker to support residents and their families throughout their stay. The Social Worker will help address psychosocial needs, assist with care planning, and support discharge planning.
Responsibilities
Complete psychosocial assessments for new admissions
Participate in interdisciplinary care plan meetings
Provide emotional support and counseling to residents and families
Assist with discharge planning and coordination of community services
Address resident and family concerns
Advocate for residents’ rights and well-being
Maintain accurate documentation in the electronic medical record (EMR)
Ensure compliance with facility policies and state regulations
Qualifications
LMSW or LCSW preferred (New York State)
Experience in skilled nursing, rehabilitation, or healthcare preferred
Strong communication and interpersonal skills
Ability to work as part of a multidisciplinary team
Organized and able to manage multiple cases
